It is imperative errors and near misses are learned from, and effective preventative measures are implemented (2). The Safer Radiotherapy publication series facilitates comparison of locally identified trends against the national picture. The Patient Safety in Radiotherapy Steering Group (PSRT) recommends implementing learning from this analysis locally.
